MT FAQ's

What are the prerequisites to apply for Master Trainer status?

1. Complete the training from one of the CDC recognized training entities.
2. Facilitate at least one complete cohort through the year long DPP Lifestyle Change program.
3. Submit your data to CDC in accordance with the DPRP standards.
4. Achieve or maintain your program’s full recognition status.

What is the procedure to apply for Master Trainer status?

1. Complete the application below including a letter of recommendation from a supervisor who is familiar with your work delivering the DPP.
2. Schedule an interview with State of Wellness senior staff to discuss your program delivery experience and interest in expanding DPP Lifestyle Coach training within organization.

Is there a contract agreement with State of Wellness?

Yes. once your application is approved for MT training, a contract will be issued. Unless otherwise stipulated the contract must be signed by all parties and returned to State of Wellness prior to scheduling your training. The contract is between you, your organization and State of Wellness. Details on duration, recertification and roles and responsibilities of each party are clearly defined within the contract agreement.

How often is DPP MT training offered?

Typically, we offer two MT trainings each year. We do not schedule them in advance rather base the scheduling on request from organizations seeing to have one or more of their Lifestyle Coaches trained.
